Анализ влияния соматической патологии и ее коррекции на уровень внутриглазного давления при глаукоме у пациентов пожилого и старческого возраста.</p></sec><sec><title>Методы</title><p>Методы. В исследование были включены 256 пациентов (78 (30,5%) мужчин, 178 (69,5%) женщин; 512 глаз) с впервые выявленной первичной открытоугольной глаукомой (ПОУГ) I-III стадий. Средний возраст всех пациентов составил 64,7±1,97 года. Протокол исследования объединил следующие показатели: возраст, анамнез (сопутствующая соматическая патология и ее коррекция), стадии заболевания, уровень офтальмотонуса (исходный, через 2 недели, 1 месяц, 3 месяца, 6 месяцев и через год после начала гипотензивной терапии), гипотензивный медикаментозный режим и его изменения.</p></sec><sec><title>Результаты</title><p>Результаты. Больные ПОУГ в 93,8% случаев имели сопутствующие соматические заболевания. Патология со стороны сердечно-сосудистой системы зарегистрирована у 232 (90,6%) пациентов, из них 188 (72,9%) человек получали комбинацию лекарственных средств для лечения соматических заболеваний, остальные находились на монотерапии (70 человек, 27,1%), которая преимущественно была представлена и-АПФ (41,4%) и БРА (32,9% из всей моно- терапии). Для лечения глаукомы препаратами первого выбора были β-адреноблокаторы (75,4%). Аналоги простагландинов (АПГ) назначались в 17,8% случаев. Пациенты, находящиеся на гипотензивном режиме в виде монотерапии β-адреноблокаторами, в 57,8% случаев сохранили этот режим спустя 1 год после назначения. Надо отметить, что снижение ВГД оказалось более значительным, чем заявлено в инструкции к препаратам. При этом системно для коррекции сердечно-сосудистой патологии пациенты получали монотерапию и-АПФ или БРА в 18,4% случаев. В 33,3% случаев отмечено сочетание БАБ с ТД, БРА, АК. В 17,2% случаев пациенты получали антиагреганты.</p></sec><sec><title>Заключение</title><p>Заключение. Вопрос взаимодействия лекарственных препаратов, применяемых для лечения глаукомы и других заболеваний, является актуальным в течение всей жизни пациента для достижения стабилизации глаукомного процесса, исключения или уменьшения побочных действий лекарственных средств и повышения приверженности к назначенной терапии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose</title><p>Purpose: Analysis of somatic pathology and its correction according to the intraocular pressure (IOP) level in elderly and senile patients with glaucoma.</p></sec><sec><title>Methods</title><p>Methods: The study included 256 patients (512 eyes) with newly diagnosed POAG stage I-III, 78 (30.5%) men and 178 (69.5%) women. The average patient age was 64.7±1.97 years. The study protocol combined the following indicators: age, anamnesis (concomitant somatic pathology and its correction), stages of the disease, IOP level at baseline, 2 weeks, 1 month, 3 months, 6 months and a year after the start of hyportensive therapy, hypotensive drug regimen and its changes.</p></sec><sec><title>Results</title><p>Results: Patients with POAG in 93.8% of cases had concomitant somatic diseases. Cardiovascular pathology was registered in 232 (90.6%) patients. The majority of patients received combination therapy (188 people, 72.9%), the remaining patients received monotherapy (70 people, 27.1%), which was mainly represented by i-ACE (41.4%) and ARB (32.9% of all monotherapy). For the treatment of glaucoma, β-blockers (75.4%) were the first choice drugs. Prostaglandin analogs (PA) were administered in 17.8% of cases. Patients with β-blockers monotherapy in 57.8% of cases retained this regimen a year after administration. It should be noted that the level of IOP reduction was more promineny than stated in the instructions of the drugs. At the same time, for the correction of cardiovascular pathology, monotherapy in the form of i-ACE and ARB was received in 18.4% of cases. In 33.3% of cases, patients received a combination of BAB with TD, ARB, AK.
