В настоящее время хирургическое вмешательство является наиболее эффективным способом нормализации уровня внутриглазного давления (ВГД) и сохранения зрительных функций при глаукоме. В зависимости от материала различают следующие виды дренажей: на основе коллагена; с металлической основой; с синтетической полимерной и биополимерной основами. В работе изложены основные характеристики перечисленных видов дренажей и приведены литературные данные по их применению в клинической офтальмологической практике. На основании анализа данных сделано заключение, что из-за несовершенства материалов вокруг дренажей формируется плотная соединительнотканная капсула, облитерирующая вновь созданные пути оттока внутриглазной жидкости. Большой выбор дренажей, существующий в настоящее время, говорит об отсутствии материала, вокруг которого не формировалась бы рубцовая ткань. В настоящее время наиболее сбалансированными свойствами обладают синтетические дренажи из природных полимеров. Они сочетают в себе достоинства всех групп: большую стойкость по сравнению с природными полимерами, высокую биосовместимость, физиологический транспорт внутриглазной жидкости. Однако и они не соответствуют всем необходимым критериям, что обуславливает дальнейший научный поиск в этом направлении.</p></abstract><trans-abstract xml:lang="en"><p>Glaucoma is one of the main diseases leading to visual impairment and blindness. At present, drainage device surgery is the most effective way to normalize the intraocular pressure and preserve visual functions. Depending on the material, the following types of drainage devices are distinguished: collagen-based; metal-based; synthetic polymerbased and biopolymer-based. This paper described the main characteristics of these types of drainage devices and provides data on their use in clinical ophthalmic practice. Data analysis led us to believe that due to the imperfection of the materials a dense connective tissue capsule forms around the implant, obliterating the newly created outflow pathways of the intraocular fluid. A wide selection of drainage devices existing at present indicates the absence of material around which scar tissue would not form. At present synthetic drainage device from natural polymers shows optimal properties. They combine the advantages of all groups: greater resistance compared to natural polymers, high biocompatibility, physiological transport of intraocular fluid.
