Оценка эффективности и безопасности повторной транссклеральной микроимпульсной циклофотокоагуляции (мЦФК) у пациентов с рефрактерной оперированной глаукомой (РГ).</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ. Обследовано 89 пациентов в возрасте 74,2±7,3 года с развитой (16), далекозашедшей (58) и терминальной (15) стадиями некомпенсированной первичной открытоугольной глаукомы (ПОУГ) до и в течение 12 месяцев после выполнения первой мЦФК (прибор SUPRA 810, «Quantel Medical», Франция) с применением стандартных параметров лазера — 100 Дж. Показания к повторной мЦФК определены у 23 пациентов через 3 (1 пациент), 6 (14 пациентов), 9 (8 пациентов) месяцев после первой процедуры. Повторная мЦФК проведена с большей энергией воздействия — 125 Дж.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. После первой процедуры гипотензивный эффект достигнут у 66 (74,2%) больных с РГ до 12 месяцев наблюдения. Проведение повторной мЦФК у 23 пациентов позволило снизить ВГД при развитой на 31,2%, при далекозашедшей на 31,8% и при терминальной стадии на 22,9% (p&lt;0,05) к 6 месяцам наблюдения. В итоге в течение 12 месяцев наблюдения мЦФК (однократная и двукратная) привела к стабилизации ВГД в 83,1% случаев.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Однократная и повторная микроимпульсная ЦФК с лазерной энергией 100 и 125 Дж является эффективным и безопасным методом лечения пациентов с РГ. Проведение однократной мЦФК с лазерной энергией 100 Дж оказалось эффективно у 66 (74,2%) пациентов к 12 месяцам наблюдения, а после однократной и повторной (с лазерной энергией 125 Дж) – у 74 (83,1%) больных. Возможен пересмотр базовых параметров процедуры мЦФК со 100 до 125 Дж для достижения более длительного и одновременно безопасного гипотензивного эффекта у больных с РГ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>PURPOSE</title><p>PURPOSE. To evaluate the efficacy and safety of repeated micropulse transscleral cyclophotocoagulation (MP-TSCPC) in patients with previously operated refractory glaucoma.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS. We examined 89 patients aged 74.2±7.3 years with moderate (16), advanced (58) and terminal (15) stages of uncompensated primary openangle glaucoma (POAG) before and within 12 months after the first MP-TSCPC (SUPRA 810, “Quantel Medical”, France) using standard laser parameters — 100 J. Indications for repeated MP-TSCPC were determined for 23 patients in 3 (1 patient), 6 (14 patients), 9 (8 patients) months after the first procedure. Repeated MP-TSCPC was carried out with higher impact energy — 125 J.</p></sec><sec><title>RESULTS</title><p>RESULTS. After the first procedure, the hypotensive effect was achieved in 66 (74.2%) patients with refractory glaucoma lasting up to 12 months of follow-up. Repeated MP-TSCPC in 23 patients reduced the IOP by 31.2% in moderate, 31.8% in advanced, and 22.9% in terminal stages (p&lt;0.05) by 6 months of follow-up. As a result, during 12 months of observation, MP-TSCPC (single and double) led to stabilization of the IOP in 83.1% of cases.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION. Single and repeated micropulse TSCPC with laser energies of 100 J and 125 J is an effective and safe method of treating patients with refractory glaucoma. A single MP-TSCPC with laser energy of 100 J was effective in 66 (74.2%) patients by 12 months of observation, and single followed by repeated (laser energy of 125 J) — in 74 (83.1%) patients.
