Определение клинико-эпидемиологических особенностей развития и прогрессирования первичной открытоугольной глаукомы (ПОУГ) у пациентов с отягощенным семейным анамнезом.</p></sec><sec><title>МЕТОДЫ</title><p>МЕТОДЫ. Работа является частью исследования, в котором приняли участие 6 клинических баз регионов РФ. Включены данные 134 человек (134 глаза), из которых 51 (38%) мужчина и 83 (62%) женщины. В группу 1 (60 человека, 60 глаза) вошли пациенты со спорадической глаукомой, в группу 2 (56 человек, 56 глаз) — пациенты с наследственно отягощенной глаукомой. Здоровые лица (18 человек, 18 глаз) составили контрольную группу. Средний возраст всех пациентов составил 68,1 (62,6; 72,4) лет. Выполняли рутинные и специализированные методы обследования на глаукому.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Средний возраст пациентов со спорадической глаукомой при диагностировании ПОУГ составил 64,5 (59; 67,9) года, пациентов со отягощенным анамнезом — на 5,9 лет меньше, 58,6 (54,6; 61,5) лет. Статистически значимыми в группах 1 и 2 были различия в средней толщине комплекса ганглиозных клеток сетчатки — 26 (24; 28) и 25 (22; 27,5) мкм; глубине залегания решетчатой пластинки — 450,5 (360; 585) и 512,5 (437; 631) мкм; расстоянии «мембрана Бруха – внутренняя пограничная мембрана» (198,5 (163,5; 265,5) и 155,5 (110; 225,5) мкм) и толщине сетчатки в макуле (274,5 (261; 286) и 262 (246,5; 273) мкм). Пациенты с отягощенным анамнезом значительно меньше удовлетворены своим текущим состоянием (63,96 баллов), по сравнению с пациентами со спорадической ПОУГ (83,51 баллов).</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Профилактические осмотры у лиц с отягощенным семейным анамнезом следует проводить в более раннем возрасте. При этом нельзя недооценивать оптическую когерентную томографию как один из наиболее чувствительных методов обследования, особенно на ранних этапах заболевания. Пациентам с наследственно отягощенной ПОУГ следует рекомендовать консультации психолога с целью улучшения психоэмоционального состояния.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>PURPOSE</title><p>PURPOSE. To identify clinical and epidemiological features of the course of primary open-angle glaucoma (POAG) in patients with a verified family history of the disease.</p></sec><sec><title>METHODS</title><p>METHODS. This work is part of a multicenter study conducted across six clinical sites in different regions of the Russian Federation. The analysis included data from 134 individuals (134 eyes), of whom 51 (38%) were men and 83 (62%) were women. Group 1 (60 patients, 60 eyes) comprised individuals with sporadic glaucoma, while group 2 (56 patients, 56 eyes) included patients with hereditary glaucoma. The control group consisted of 18 healthy subjects (18 eyes). The mean age of all participants was 68.1 (62.6; 72.4) years. Standard and specialized glaucoma examinations were performed.</p></sec><sec><title>RESULTS</title><p>RESULTS. The mean age at POAG diagnosis in patients with sporadic glaucoma was 64.5 (59; 67.9) years, whereas in patients with a burdened family history it was 5.9 years younger, amounting to 58.6 (54.6; 61.5) years. Statistically significant differences between groups 1 and 2 were found in the mean thickness of the ganglion cell complex — 26 (24; 28) µm and 25 (22; 27.5) µm, respectively; lamina cribrosa depth — 450.5 (360; 585) µm and 512.5 (437; 631) µm; Bruch’s membrane to inner limiting membrane distance — 198.5 (163.5; 265.5) µm and 155.5 (110; 225.5) µm; and macular retinal thickness — 274.5 (261; 286) µm and 262 (246.5; 273) µm. Patients with a burdened family history were significantly less satisfied with their current condition (63.96 points) compared to the patients with sporadic POAG (83.51 points).</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION. Preventive screening in individuals with a burdened family history should be initiated at an earlier age. Optical coherence tomography should be regarded as one of the most sensitive diagnostic tools, particularly at early disease stages.
