Влияние местной гипотензивной терапии на состояние тканей глазной поверхности и исход антиглаукомных операций у пациентов с первичной открытоугольной глаукомой

Аннотация

На сегодняшний день глаукома является ведущей причиной необратимой слепоты. Единственным фактором, на который можно воздействовать и тем самым предотвратить потерю зрительных функций, является уровень внутриглазного давления (ВГД). Как правило, снижение офтальмотонуса начинают с местной медикаментозной терапии, которая, по некоторым данным, может неблагоприятно воздействовать на глазную поверхность и ухудшать прогноз возможной фистулизирующей хирургии. Причина этого заключается в том, что длительное применение различных гипотензивных капель, особенно с консервантом в составе, приводит к необратимым изменениям глазной поверхности, индуцирует хроническое воспаление, способствующее рубцеванию созданных путей оттока, что ограничивает и в конечном итоге блокирует отток водянистой влаги из передней камеры. При этом установлено, что бесконсервантная терапия продолжительностью менее 6 месяцев не оказывает негативного влияния на исход антиглаукомных операций (АГО). Поэтому, назначая местное гипотензивное лечение, следует помнить, что использование большого количества режимов с разнообразным сочетанием препаратов, пренебрежение к медикаментам без консерванта ухудшают прогноз будущей хирургии, зачастую неизбежной. Особенно важно это учитывать у пациентов с ожидаемо высокой продолжительностью жизни, у которых уже на старте рационально выбирать в качестве первого режима не содержащие консерванты капли и проводить АГО (минимально инвазивные хирургические вмешательства) как можно раньше.
